Whether you are in the transition or considered totally electronic, paper still plays a role in the everyday processing of PHI. Outside sources continue to send paper PHI, archived records are in paper format, and in many places, paper is still being generated by the healthcare facility.  This all creates a hybrid environment. To deal with this and incorporate paper into the EHR one must digitize these documents.  This process can be an arduous task.  This is expensive and time-consuming, but necessary for a complete EHR.
